Release to Parent Form
MCHS students are provided transportation to and from all school events. Parents/Guardians: submit a "Release to parent form", at least 2 days prior to the trip.  The sponsor will receive a notification of the request.   

MUSIC REQUEST FORM
To make a request for your favorite (school-appropriate) music, click on the music request form link.  Submitting a request does not guarantee your song will be played. The following guidelines must be met:
1. Song must be less than five (5) minutes in length.
2. Song must contain school-appropriate lyrics; no foul language will be accepted.
3. Please refrain from requesting the same song more than once.
4. Follow @CoachKwaz32 on Twitter if you wish to know the name of a previous song.

Fill in the information and make sure you include an email address under the contacts
select complete registration
Under My Calendars, click manage
Add "your activity" Club
