Nurse's Office

A registered nurse is available at both campuses to assist with student health concerns.  The school nurse supports the health and safety of all students, particularly those with chronic or severe medical conditions. All students who sustain an injury during class or on school grounds must report immediately to the nurse for evaluation and care.  If a student feels ill and wishes to leave school, they must first see the registered nurse to be assessed before leaving. This ensures proper care and safety protocols are followed.  At the South Campus, if a student cannot safely use the stairs due to an injury or health condition, they may obtain an elevator pass from the nurse. However, an elevator pass is only granted with a medical accommodation note from a physician.  The South Campus nurse may issue a one-time daily elevator pass for temporary situations. Certificate of Child Health Examination

In accordance with the Illinois School Code,[77 Ill. Adm. Code 665] all students entering as freshmen or transferring into an Illinois school must complete a physical examination performed by a licensed physician (M.D. or D.O.), an Advanced Practice Nurse (APN), or a Physician Assistant (PA). This physical exam must be documented on the Certificate of Child Health Examination form, which is provided by the Illinois Department of Public Health (IDPH). Key Requirements:

  1. Physical Exam: The Certificate of Child Health Examination must be submitted by the first day of school or uploaded into PowerSchool during the registration process for all freshmen and transfer students. Please ensure the form is completed in its entirety, including the Parent/Guardian section. This helps ensure that all required medical information has been provided.

* An IESA/IHSA sports physical form will not be acceptable as a 9th-grade physical.

  1. Immunizations: Documentation of immunity to or vaccination for the following diseases is required: Tdap (Tetanus booster), Dtap (Diphtheria, Pertussis-whooping cough, Tetanus), Meningitis, Polio, MMR (Measles, Mumps, Rubella), Hepatitis B, and Varicella (Chicken Pox).  Note: If a student has a history of varicella disease (chickenpox), verification from a healthcare provider can be submitted in lieu of the vaccination.

  2. Seniors: A second meningococcal vaccine is required prior to the start of the senior year for compliance. This dose must be administered on or after the student’s 16th birthday. 

  3. Dental: According to Illinois Statutes Section 27-8.1 [105 ILCS 5/27-8.1] of the School Code, all students in 9th grade are required to have a dental examination completed by a licensed dentist.    

Compliance and Exclusion: Per the Illinois School Code, no student will be allowed to attend school unless they have submitted the required physical examination documentation and proof of all necessary immunizations. Failure to comply with these requirements by October 15th will result in the student being excluded from school attendance until all required documentation is provided.  Please ensure that all forms are completed, signed, and submitted by the required deadline to avoid any disruption to your child’s education. 

9th-grade Dental Notice: Proof of this examination must be submitted to the school no later than May 15 of the current freshman school year. Please note that if proof of the dental examination is not received by May 15, the school may hold your child’s report card until one of the following is provided:
(i) proof of a completed dental examination, or
(ii) proof that a dental examination is scheduled to take place within 60 days after May 15.

Vision and Hearing Screening Requirements  

Per the Illinois School Code, vision and hearing screenings are conducted for all freshmen during their physical education semester and for newly enrolled students. School vision screenings do not replace a comprehensive eye exam by a licensed optometrist or ophthalmologist. Students are exempt if a completed eye exam report from a licensed provider within the past 12 months is submitted to the school.

Emergency Action Plans: For students with medical conditions such as Asthma, Diabetes, Seizures, or Severe Allergies (requiring an EpiPen or other emergency medication), an Annual Emergency Action Plan must be completed and submitted for each school year. This plan is necessary to ensure the immediate and appropriate care of your child in the event of an emergency. Having a current Emergency Action Plan on file helps school staff respond quickly and effectively to medical emergencies, ensuring the safety and well-being of your child while at school.

If your student requires prescription medication during school hours, please complete the Prescription Medication Authorization Form, signed by both the physician and parent/guardian. In compliance with the Illinois School Code,[105 ILCS 5/22-30(c)], the District permits students with asthma and severe allergies to self-carry and self-administer medication. If your child will carry and use albuterol or epinephrine, a completed prescription authorization form is required.

Note: Students with diabetes who self-carry and administer insulin must have this documented in their Diabetes Care Plan.

Prescription Medication Form


Over-the-counter medications (e.g., acetaminophen, ibuprofen, allergy medications) are not provided in the health office. In accordance with school guidelines, students may self-administer (OTC) non-prescription medications. To authorize this, please complete the "Over-the-Counter Medication Authorization" form in PowerSchool.  The link is under the "General" tab in the Student Forms section.  

Undesignated Standing Orders for Illinois Schools

Under applicable Illinois Department of Public Health (IDPH) standing orders and the Illinois School Code, schools are authorized to maintain and administer undesignated medications for emergency use. This includes stock albuterol for asthma-related respiratory distress, epinephrine auto-injectors (EpiPens) for severe allergic reactions, and opioid antagonists (e.g., naloxone) for suspected opioid overdoses. These medications may be administered by trained school personnel in good faith during emergencies, in accordance with established protocols and applicable state laws.[105 ILCS 5/22-30(c)].

Concussion Information

School nurses and athletic trainers play critical roles in managing student concussions. Clear communication between the school nurse, athletic trainer, parents, and healthcare providers is essential for effective concussion management.
