School Physical & Immunizaation Requirement for All Incoming Freshmen
This is a reminder that all 9th graders must have a current physical and immunization record on file in order to attend school.  Any student who does not have a valid physical on file by the first day of school in August will be excluded from school until such time that it is received.  Additionally, students must have a physical turned in on or before the registration day in July (date TBD) in order to receive his or her schedule that day.

If you have not done so already, you are encouraged to schedule an appointment now for your student’s physical.  Health care providers’ schedules usually book up quickly, and appointment cards for scheduled appointments are not accepted for school attendance or schedule release.

Valid physicals must be dated 8/15/16 or later for school attendance.  If your student will be in a sport, physicals are valid for 13 months, so keep that in mind when scheduling the appointment.  Please contact me via email at jpfieffer@mchs.net if you have any questions.

Physicals can be turned in anytime to either main office (South Campus or Central Campus), so go ahead and drop them off as soon as you have them.  If you would like to mail it, please FIRST MAKE A COPY TO KEEP AT HOME, and then mail the original to:
