Welcome to the Nurse's Office

A registered nurse is available at both campuses. All students who receive injuries in classes or on the school grounds should report to the nurse immediately. All students requesting permission to leave school due to illness must see the registered nurse prior to leaving school. At the south campus when a student is unable to walk up or down stairs because of a health problem or injury, an elevator card/key can be obtained from the nurse. If a student fails to return the card/key when it is no longer needed, or if the key is lost, then a fee will be assessed to the student.

High School Registration

Students in grades 9-12 register at the high school. To find out more information about registration requirements, please call the main office at MCHS 815-467-2140. Please complete the registration packet and contact the school to arrange a time to enroll the student. There may be additional paperwork for you to complete at the school. Certificate of Child Health Examination
2009-2010

All students entering as freshmen or as transfer students must have a physical exam form completed and ready to turn in the first day of school. State law also requires that all students entering school be immunized against polio, diphtheria, tetanus, pertussis (whooping cough), measles, mumps, and rubella. In addition, all students must show proof of receiving the Hepatitis B vaccination series. Proof of immunity is verified by documented evidence by a health care provider of having received the vaccine or of having had the disease. State law provides that students who do not meet these requirements be excluded from school. Student transferring from another state must have a current physical completed on an Illinois comparable health form on their first day of school.

Vision and hearing screening will be done, as mandated, for freshmen during the semester in which they have physical education and for new students in the school district. Students will not be required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months and that evaluation is on file at the school

Self-Administration of Asthma Medication
2009-10

Pursuant to the School code, the School District will permit the self-administration of medication by a student with asthma, if the downloaded document is completed by the student's parents or guardians and physician.
